Procedure for Ordering Pathology Supplies
 
Celligent Diagnostics is happy to provide all supplies required to submit specimens.  You may download a printer friendly version of our Supply Order Form on this website by clicking here.
 
Supplies may be ordered by:

1.      Faxing the Supply Order Form to Celligent Diagnostics at 704-549-0559
2.      Leaving the Supply Order Form in your courier’s pick-up box
3.      Calling Celligent Diagnostics at 704-549-8444, ext. 101
 
Your supply order will be delivered as soon as possible, generally within 24 hours.
Remember that additional time is needed to personalize your requisition forms.
